This is the standard waiver for all activity sessions, covering safety and general liability.

Minors & Youth Participation

Important: If a parent or guardian brings a minor (under 18) to an activity—and you as the coordinator have permitted their attendance—this specific form must be signed by the legal guardian. The standard adult waiver is not sufficient for participants under 18.
